Guides - Three-part form
A form in three parts that follows the customer before, during and after the treatment. Turned on by Gecko support.
Three-part form
A three-part form combines three forms in one: a questionnaire the customer fills out when booking, a result form you fill out on the booking, and a satisfaction form the customer fills out afterwards. That way the advance information from the customer, your own notes on the treatment and the customer's assessment are collected in one form, e.g. in a course of treatment with a physiotherapist. Three-part forms are not turned on by default. Contact Gecko support if you want to use them. For most purposes a regular form is enough (see Create new questionnaire).
How it worksWhen three-part forms are turned on, you choose the type when you create the form under Settings > Questionnaires > Questionnaire settings:
(1) Under 'Type' you choose (2) 'Three-part form (questionnaire/result/satisfaction)'. When you create the questions with 'Edit questions', you choose for each question which part it belongs to:
(1) 'Position':
With 'Show preview' you see each of the three parts separately. The form is attached to a service under 'Form' in Service settings. If you want the PDF report of the form included in your own copy of the confirmation e-mail, tick 'Attach PDF' on the tab 'Forms' under 'Front-end' in Settings > General > Advanced settings. |
